Drug Therapy Management (DTM) and Medication Therapy Management (MTM) are medical services provided by a pharmacist whose goal is to optimize drug therapy and improve therapeutic outcomes for patients. DTM/MTM includes a broad range of professional services, including performing a patient assessment and/or a comprehensive medication review, formulating a medication treatment plan, monitoring the efficacy and safety of medication therapy, enhancing medication adherence through patient empowerment and education, and documenting and communicating DTM/MTM services to prescribers in order to maintain comprehensive patient care.  These medication reviews include analysis of drug-to-drug interactions with your prescription medications, OTC medications, and supplements.

Pharmacogenomics

Pharmacogenomics is an advanced practice where a genetics test can be ordered to determine how an individual’s body may respond to medication. A specially trained pharmacist will review and interpret the information and work with an individual’s physician team to recommend any changes if applicable. Pharmacogenomics uses genetic information to individualize medication therapy, moving away from the “one size fits all” model. Geritom uses cutting edge evidence-based guidelines to help with many difficult to treat disease states from psychiatry, neurology, and other specialties to optimize health outcome.

According to a study by the National Community Pharmacist Association patients who enroll in a medication synchronization program average more than 100 additional days on therapy per year and are 30% more likely to take their medications as prescribed.  90% of patients with synchronized medications were deemed adherent as measured by Proportion of Days Covered (PDC) as opposed to just 56% of patients not receiving synchronized refills."
